  3.               ENROLLMENT APPLICATION AND AGREEMENT

  5.     Upon acceptance of this agreement by ISA, I agree that:
  6.     
  7.     1.   As an Independent Representative of ISA, I am and shall remain
  8.     a self-employed independent contractor, and I am not an employee or
  9.     agent of ISA;
  10.     
  11.     2.   I will operate, be engaged in, and be responsible for my own
  12.     business; I will promote the sales of ISA's electronic advertising
  13.     and other products, and wil abide by all ISA's rules, regulations,
  14.     policies and procedures, as may be amended from time to time,
  15.     including those set forth in the ISA Independent Representatives'
  16.     Guidelines, Training Manuals, Compensation Plan, etc., which are
  17.     hereby incorporated herein by this reference.

  19.     I acknowledge that I have read, understand, and agree to abide by
  20.     all the provisions set forth in this agreement.  I understand I am
  21.     under no obligation and there are no requirements beyond my filing
  22.     this application and agreement and my payment of a $19 annual fee. 
  23.     I am under no obligation to make any other purchase whatsoever from
  24.     ISA, and any purchases of literature, sales aids, supplies or any
  25.     product from ISA is strictly optional on my part.
